Mike McGaughrin has left his position as group operations director at Tenet as part of a continuing restructure at the top of the company.
In his role, he was responsible for IT, HR, operations, customer services, compliance operations and change management and had been with the company since 2006.
McGaughrin joins a list of other high profile departures from Tenet in 2011, including founder and CEO Simon Hudson.
He stepped down in July, saying he could not commit to five years asked of him, and a successor has not yet been appointed.
Meanwhile, Peter Lane, Tenet's former commercial director, left in January to "pursue opportunities elsewhere".
Iain Black also recently left the board after he departed from Aegon, one of the providers which part-owns the group.
Graeham Sampson joined as group finance director in April.
